  To register:  Print and complete the form and mail it to the Voter Registration Clerk at the above address.  All applications must have an original signature.
   
  Under a new FEDERAL law, if you submit your application to register to vote through the mail and you are a new registrant that has not voted in this County, you must present this office with a copy of a personal identification document.  DO NOT MAIL THIS OFFICE YOUR ORIGINAL COPY OF ANY OF THESE DOCUMENTS!!!  Your name and current 911 residence address must be printed on the document.  You may use one of the following:  current and valid photo identification, such as your Indiana driver's license; current utility bill; current bank statement; current government check; current paycheck; or any other government document that shows your name and address.
It is important to fill out the entire card and to include your signature.  Indiana Election law will not allow your application to be processed until all the necessary information is received. 
A Voter Identification Number is also necessary.  This number is the 10 digit number on your Indiana driver's license.  If you do not have an Indiana driver's license then you may submit the last four digits of your Social Security Number.  You must answer the questions concerning US Residency and being age 18 before the next election.
A voter can only be registered to vote in one location.  Please be sure to list your previous voter registration address, or name change, where applicable on the voter registration card.  This information helps eliminate duplications in a jury pull and for voting purposes.  If you are a college student, you must choose between registering in the County and State of your "college residence" or registering in the County and State of your "home residence".
